*U6 requires child's birthdate between 8-1-2004 and 7-31-2006
Season begins September 7th (runs for 6 weeks)

1) at Back To School Night (Smithville Primary) on August 12th (4:30 - 6:30pm) or,
2) Print off form HERE complete, and mail to:
